Clinical Social Work Case Manager

32 hours/week

Position Description

Clinical Social Work Case Manager for older adults who live in Hearth housing. Responsibilities include assessment, case management, interdisciplinary care planning, crisis intervention, maintaining clinical chart documentation, and brief counseling. Reports to the Director of Behavioral Health. Previous work experience with at-risk older adults and persons with substance use and chronic mental illness preferred. Ability to work independently in a fast-paced environment, and with interdisciplinary team essential. Minorities and bilingual Spanish speakers are strongly encouraged to apply.

Qualifications

MSW with Massachusetts license, LCSW or LICSW.

Location

Various Hearth sites in Boston.

Compensation

Commensurate with experience.
