Outreach Prevention Case Manager

Full-time

Position Description

Engage with and assess at-risk elders in the areas of housing barriers, mental health, substance abuse, cognitive skills, and physical health problems. Provide case management and housing advocacy; assist in making the transition to appropriate community residences and resources. Assist with connection to mainstream benefits, services, and intervention. Provide counseling and stabilization services to those obtaining housing and those at risk of homelessness, including home visits. Provide outreach in Boston area shelters weekly. Collaborate regularly with a network of homeless services providers. Weekly team supervision provided.

Qualifications

Attributes/Skills: Professional, reliable, independent, collaborative, resourceful, patient, assertive, respectful, adaptive, and compassionate. Verbal and written communication. Tech savvy. Strong passion for social justice and alignment with our agency mission and values.

Prior Experience: Two years prior social service experience working with the homeless, elders, and/or underserved populations.

Education: Bachelor’s degree preferred.
