Social Innovation FormThe Social Innovation Form named Hearth's Outreach Program a social innovator for its unique approach to addressing elder homelessness in the Greater Boston area. The organization won the Social Innovation Forum’s The Promise of Healthy Aging category, which was co-sponsored by Tufts Health Plan Foundation.

2006: Outstanding Community Service Partner

2006: 2006 Leadership Award
Each year the National Alliance to End Homelessness recognizes one government agency, one private corporation, and one nonprofit for their "achievements in the struggle to end homelessness."  Hearth received the Leadership Award for the nonprofit sector.
2005: Sixth Annual Healthcare and Aging Award
American Society on Aging LogoThe American Society on Aging presents this award to organizations who have demonstrated high-quality innovative programs that enhance the health-related quality of life in older adults.
2004: Out of the Blue Grant
This award - rarely given out by The Boston Foundation - recognizes “organizations that have demonstrated a high level of achievement in terms of strong and effective community leadership, outstanding quality of work, and an impressive history of accomplishment.”

2001: Fannie Mae Foundation Maxwell Award
Fannie Mae Foundation Maxwell AwardChosen as one of 12 finalists to receive the Maxwell Award for Excellence from the Fannie Mae Foundation. The award is for the production of affordable housing, in particular, the Ruth Cowin House, a home for nine low-income elders in Brookline. (link opens a PDF)
